基于遗传算法的临床路径本地化研究

摘要: 由于国家标准临床路径的表单项需要适应不同医院的各种情况,不能直接用于具体医院的临床路径管理工作。鉴于此,提出一种基于遗传算法的临床路径本地化算法。该算法以国家标准临床路径和医院住院病人的完整诊疗过程数据作为输入,生成医院本地化的临床路径。建立了国家标准临床路径电子化模型,将病人诊疗过程中的收费项目与标准临床路径中的表单项的映射关系编码,通过遗传算法搜索出适应度最佳的映射关系编码,并用该结果将国家标准临床路径各阶段的表单项转化为具体的临床诊疗过程收费项目集合,得到本地化临床路径。实验结果表明所提算法能够得到准确度高、通用性强的本地化临床路径,能够有效地辅助我国医疗机构进行本地化临床路径的制定。

Abstract: Most of the national standard clinical pathways cannot be directly used to guide clinical path management in medical institutions due to the adaption of different situations in different hospitals.Aiming at this problem,a customization method of clinical pathway based on genetic algorithm was proposed.The method could generate the customizing clinical pathway by taking the national standard clinical path and inpatient care process data as the input.The national standard clinical pathway electronic model was established,and the mapping relationship between the charging items in inpatient care process data and the form items in standard clinical path was encoded.The best-fitting mapping relationship code through the genetic algorithm was searched,and the national standard clinical path at each stage was converted into a collection of specific clinical diagnosis and treatment process charging items with this searching result.Thus a customized clinical path was obtained.The experiment results showed that CPLGA algorithm was a efficient way of generating more precise clinical pathways and it was very versatile which could effectively assist the development of customization of clinical pathways in Chinese medical service provider.
